The wheels are designed to spread out at a 5 degree angle - this is typically stable enough for any dog, making training wheels unnecessary. Did I understand your question correctly? Perhaps you're speaking about a front wheel cart?

We are finding that the harness should be tight across the top of the wheelchair -- not loose and hanging down. In addition, males can be given a little extra room where they need it by leaving some space between the leg rings.
